The Denplan agreement is between you and your current dentist and therefore non transferable. If you wish to change your dentist you will need to terminate this contract and ask your current dentist to fill in a Denplan leaving form. Once in position of the leaving form you can telephone the practice and arrange an initial consultation appointment. Your initial consultation appointment is free (as long as you bring your leaving form) and as long as you are deemed dentally fit we will simply complete new Denplan registration forms and the transfer is complete.
